Complimentary Initial Consultation

Initial consultation to discuss your concerns and individual situation to assess whether how I can be help.

Support with Disability and Carer’s Allowance Applications

After our initial phone call, I will read all professional reports and EHCP/ISPs if these are in place. Using statements from within these reports in conjunction with the Local Authority’s graduated response I will complete the DLA Form in draft form in a PDF format. This will be returned to you electronically for your approval and any amendments changed. On your approval the completed DLA with copies of all professional reports as supporting evidence will be sent in a paper format ready for you to sign and post. You will also be sent electronic copies for you to amend for future applications.

Support with Personal Independence Payment (PIP)

After our initial phone call, I will read all professional reports and EHCP/ISPs if these are in place.  Using statements from within these reports I will complete the PIP format online.  This form cannot be completed in draft as it has to be handwritten which I can do as your representative.  The original will be sent to you for signing and posting, with a copy for you to keep for your records.

Full review of all outside Professional Reports and Individual Support Plans if applicable

Support and advice in relation to Surrey’s SEND Graduated Response, I request all documents are sent to me 2 weeks before our appointment. I will provide a written planned approach and meet with you to discuss next steps as well as write an email for you outlining your requests to your child’s setting/school. This meeting will also give you a deeper insight into mainstream curriculum levels your child is being assessed against.
This is an essential part of my service and without all the background information I cannot advise next steps.

A Send Support Arrangement is an important document used to support children in the learning environment with outside professionals involved, individual support plans, risk assessments in place and/or as part of the process in requesting a Statutory Assessment as part of an EHC request.  This is a document that needs parental input as well as all outside reports if in place.
